Why isn't WP-Transformer creating the page template for me?

Solution If you're not seeing a message appear at the top of the WP-Transformer panel after files upload that says "New Template "templatename" has been created successfully." as you should be then something is wrong.
I recently checked out a couple of user's sites that had this issue and this is what I discovered.

I went through the process of preparing the index.html file plus images for the wplocalbiz theme which I know works well as I just uploaded via WP-Transformer yesterday.

Here's what's happening:

WPTF is preparing the converted index.html (converted to templatename.php) and same for the images folder ("templatename-images") but they are not getting uploaded to the server which is quite clear from the absence of any "templatename.php" template in your page templates library.

I thought it could be the theme so before deactivating the current installed theme I thought I would install the plugin "widget saver" which saves widget settings for any current theme (essential if you change themes around)... and I was confronted with an FTP server permissions block that said:

"To perform the requested action, WordPress needs to access your web server. Please enter your FTP credentials to proceed. If you do not remember your credentials, you should contact your web host."

Herein lies your problem I suspect as the plugin is attempting to access your theme folder and most likely getting stopped via an FTP permissions block.

You need to either lift this manually or get your host to lift it.
It may be a good internal safety mechanism but in my opinion (and I am big on WP security issues) it isn't necessary and it just presents problems.
